Essential Job Functions:
- Employs a status-neutral approach to become more people-focused, address HIV healthcare disparities, and facilitate access to care and other services.
- Engages with medical providers and facilitates case conferences to enhance a holistic approach to care coordination.
- Works directly with clients living with HIV/AIDS using strengths-based, stigma-free approaches that support their holistic health needs.
- Develops and completes Individualized Service Plans (ISPs) and coordinates with internal and external services to promote sexual, medical, and behavioral health, housing stability, access to community resources and benefits, and enrollment in pharmacy discount programs.
- Provides excellent engagement with clients by meeting with them on a regular, ongoing basis, which includes home visits and shelter visits, as needed.
- Ensures continuity of care through ongoing assessment of the client’s and their family members’ needs and personal support systems.
- Meets the goals of Medical Case Management to ensure that clients have timely access to comprehensive medical care and social services; prevents disease transmission and delay of HIV progression through adherence to medication strategies.
- Promotes cultural affirming care and supports client independence and self-sufficiency.
Responsibilities:
- Conducts an initial comprehensive assessment of clients’ needs and support systems.
- Coordinates services with internal and external partners required to implement the service plan, including the provision of information and effective referral services.
- Monitors client’s adherence to program guidance.
- Maintains individual files, including all written documentation related to essential functions, such as case notes, service plans, and assessments, in accordance with funding sources and agency guidelines.
- Maintains meaningful involvement with clients, family members, community partners, housing, medical providers, and other mainstream resources.
- Records data for program outcomes and ensures client files are complete, accurate, and frequently updated.
- Monitors progress through phone calls, home visits, and effective case management appointments.
- Participates in internal program team meetings to ensure coordinated efforts are in place to continue stabilization of clients.
- Represents the organization in community meetings and other networking opportunities.
- Maintains ongoing communication within the client services teams to ensure client’s needs are being met holistically.
- Other duties, as assigned.
Qualifications:
Bachelor’s degree in health, human services, or related field, and at least 2 years of experience in medical case management. Individuals without completion of a bachelor’s degree will be considered if demonstrated medical case management experience with a variety of Ryan White programs.
Job Knowledge and Skills: This position requires knowledge and sensitivity to HIV/AIDS and other target populations (LGBTQ+, homelessness, mental illness, substance abuse) along with the ability to work in a clinical and community environment. Must possess the ability to identify and communicate effectively with members of the target population. Ability to work with multicultural and diverse population required. Effective oral and written communication skills required. Excellent organizational, decision-making, problem-solving, and time management skills are required to handle multiple activities related to program implementation. High energy level and self-motivated. Knowledge of case management “best practices” and how they apply to the programs we provide. Computer knowledge should include Microsoft applications (Word, Excel, Outlook). Bilingual English/Spanish fluency is required.
Physical Requirements:
Must have reliable transportation and acceptable driving record. Transporting of participants is required.
This work requires the following physical activities: walking, sitting, hearing/seeing, talking in-person or over the phone. Occasional walking, bending, standing, and driving are needed. Work is performed in an office setting, and in home environments. Work is also performed in social and community settings, as needed. Minimal to moderate lifting is required at times. Extended work schedule may apply to accommodate client’s needs.

Salary: Non-exempt, full-time position with pay of $48-50k based on experience and qualifications. Comprehensive benefit package including PTO, 403B, Medical and Dental coverage.
Preferred locations: This is an in-person position required to be on-site and conduct field work; applicant must live within 40 miles of LGBT Life Center site in Norfolk, Virginia.
